HRT Cafe, an anonymous platform providing tutorials and information on self-administering hormones, shut down earlier this week. The closure highlights the ongoing challenges faced by the transgender community in the UK when accessing gender-affirming care.
The website, known for being one of the largest databases for ‘DIY’ HRT in the UK, was developed by a small group dedicated to offering guidance on various masculinising and feminising medications. Self-medicating, which entails acquiring and using hormones without medical oversight, has become increasingly prevalent among trans adults. This practice is often seen as a last resort due to prolonged waiting times for NHS services and high costs associated with private prescriptions, which can reach hundreds of pounds monthly.
According to a report from TransActual in 2022, approximately 25 percent of the UK trans adult community has engaged in self-medication at some point during their transition. HRT Cafe served as a critical educational resource, detailing the safety considerations and potential side effects of various medications, while also listing online vendors supplying these treatments.
The developers of HRT Cafe announced the unexpected shutdown without any formal statement, indicating that they would no longer accept donations. This decision came shortly after right-wing publications, including The Daily Signal, reported on ‘DIY’ hormone vendors, referencing HRT Cafe and similar sites. Gender-critical commentators on social media suggested that this closure may be part of a broader campaign to target additional resources for the trans community.
While several LGBTQ+ and trans advocacy organizations caution against self-medicating due to risks associated with unregulated vendors, many individuals see it as the only accessible means to obtain life-saving medication. Samathy, a 29-year-old trans woman, shared her experience with PinkNews last year, stating that self-medicating was her only option after her general practitioner abruptly revoked her prescription. “I’d much rather enjoy being treated for my NHS-diagnosed medical condition by an NHS doctor,” she noted, emphasizing that limiting access to healthcare does not eliminate the need for it.
The issue of prescription cancellation has become increasingly concerning, as several general practitioners have cited a “lack of expertise” in providing care for trans adults. Clinical psychologist Dr Aidan Kelly criticized this stance, arguing that it undermines the role of shared-care agreements, which allow specialists to guide general practitioners in hormone prescribing. “GPs have expertise in prescribing hormones; they do it for cis people all the time,” Dr Kelly remarked.
As discussions continue around hormone access, NHS England recently announced an evidence review concerning the effects of hormones on transgender adults. This has raised further concerns about the potential for restricted access to necessary treatments.
